Patient's Rights and Responsibilities

As a patient at the Health and Wellness Center, you have the following:

Rights:

  • You have the right to quality health care.
  • You have the right to be treated with respect, consideration, and dignity.
  • You have the right to be treated in a manner that does not discriminate against any person because of age, ancestry, color, disability or handicap, national origin, race, religious creed, sex, sexual orientation or veteran’s status.
  • You have the right to request a specific health care provider at the Health and Wellness Center and to change primary or specialty providers if you so desire.
  • You have the right to participate in decisions concerning your health care.
  • You have the right to accurate information concerning diagnosis, evaluation, treatment, and prognosis of an illness or health-related condition. When it’s medically inadvisable to give such information to a patient, the information is provided to a person designated by the patient or to a legally authorized person.
  • You have the right to a second opinion or referral.
  • You have the right to privacy during medical care and during check-in.
  • You have the right to have interpretation services. when needed we will supply the Language Line for our students.
  • You have the right to the confidentiality of all communications with staff and confidentiality of the medical record, and the right to approve or refuse the release of information to the fullest extent provided by law.
  • You have the right to refuse recommendations for medical care.
  • You have the right to refuse to participate in any research program.
  • You have the right to choose the appropriate health care provider and facility that best meets your needs.
  • You have the right to provide us with a copy of your Advanced Directive or Living Will.

Responsibilities:

  • You have the responsibility to arrive as scheduled for appointments and to notify the Health and Wellness Center in advance in case of canceled appointments.
  • You have the responsibility to provide full information about your illness or health problem to assure proper evaluation and treatment.
  • You have the responsibility to ask enough questions to ensure understanding of your illness or problem, as well as your provider's recommendations for continuing care.
  • You have the responsibility to either carry out treatment and education recommendations or accept responsibility for the outcome.
  • You have the responsibility to arrange transportation home with a responsible adult and to remain with you as directed by the provider or as indicated on discharge instructions
  • You have the responsibility to become informed; through available printed material and/or discussion with staff, about the scope of basic services offered, their cost, and the necessity for additional medical insurance.
  • You have the responsibility to communicate with your health care provider if your condition does not follow the expected course.
  • You have the responsibility to make payment or arrange for payment of services.
  • You have the responsibility to maintain a healthy lifestyle.
  • You have the right and the responsibility to bring forward suggestions or grievances about the Health and Wellness Center. You may contact any of the governing board, director of student affairs, and/or the Chancellor if needed to voice your grievance. This includes the right to receive a personal response.
  • You have the responsibility to file a grievance/complaint should be submitted in writing or verbally, containing the name and address of the complainant and a brief description of the allegation of discrimination. Also see Complaints/Grievance CO13 for further information
  • You have the responsibility to conduct yourself in a respectful manner.

Insurance Information

The Health and Wellness Center is a participating provider for many insurance companies. Please contact your insurance company to verify your coverage at the Health and Wellness Center. If we are not in your network, you must request a “guest membership” from your insurance company for services provided at the center. If this is not an option for you, Penn State's Student Health Insurance Plan (SHIP) is available. Please note that even though you uploaded your insurance information to LionPATH, our systems are not connected. You must bring your insurance card to the center for your initial visit.

Accreditation

As of November 2023, the Penn State Altoona Health and Wellness Center once again achieved the milestone of Accreditation Association for Ambulatory Health Care (AAAHC) accreditation. Organizations that earn AAAHC accreditation embody the philosophy of 1095 Strong, quality every day, a transformational movement spearheaded by AAAHC to equip ambulatory leaders with the best of what they need to operate quality organizations. The three-year coverage of the accreditation allows the Penn State Altoona Health and Wellness Center to continue to review and improve the level of care afforded to Penn State Altoona students.
