Administrative Rights Request Form
Please use this printable form if you would like to request administrative privileges for your University-provided computer. This form also contains general guidelines for anyone interested in requesting administrative privileges for their University-provided computer. Please bring the completed form to the Information Technology Service Desk.
The Service Desk has a limited number of Loaner Laptops available by request for faculty and staff. They can be utilized for short term travel, meetings, or presentations. They may also be used a temporary machine when your primary desktop or laptop is being serviced. Both Dell and Mac options are available and will include a standard set of software. To request one simply stop by 230 Hawthorn, call x5356, or place a work order online. When borrowing a laptop please note the following reminders:
- A laptop request form must be signed by both the requester and the department or division head for approval before use.
- Any files saved to the laptop during use are deleted and cannot be retrieved after returning laptop.
- You are responsible for the laptop during use. Please report any issues to the OIT staff immediately.
